When making reservations, tell the agent that you need O2 for your child. They will guide you through the process. Generally what they do is send a fax to your Dr.s office who confirms that O2 is necessary and orders a prescription for the litre rate. The airlines should then notify you that the arrangements have been made. Consider following up with the airline if you don't hear in a reasonable amount of time. There is always a charge (usually $100 for every leg of the trip). Consider taking a pediatric cannula even if one is requested as they may not be provided. If O2 is needed between flights, it may be possible to do this through your Home Health Care provider.
Some airlines provided oxygen for free. Virgin Atlantic is one such example.
